Form I-134A, Online Request to Act as a Sponsor and Financial Support Declaration

ALERT: We are currently accepting submissions of Form I-134A exclusively from individuals willing to provide financial support to:

  1. Ukrainians or their immediate family members as part of the Uniting for Ukraine initiative;
  2. Cubans, Haitians, Nicaraguans, or Venezuelans, or their immediate family members as part of the Processes for Cubans, Haitians, Nicaraguans, and Venezuelans; or
  3. Colombians, Cubans, Ecuadorians, Guatemalans, Haitians, Hondurans, or Salvadorans, or their immediate family members as part of the family reunification parole processes. Only individuals who have received a specific invitation to request sponsorship and initiate the FRP process by submitting Form I-134A may proceed with the filing. An invitation number is provided in the invitation letter, and each individual submitting Form I-134A must include this number; otherwise, the Form I-134A will not be confirmed, and the FRP process will not advance.

To file Form I-134A, you must be physically present within the United States. Beneficiaries are not allowed to file Form I-134A on their own behalf.

Supporters are required to submit a separate Form I-134A for each beneficiary they agree to support, including minor children under 18 years old.

To qualify for consideration under Uniting for Ukraine, the Processes for Cubans, Haitians, Nicaraguans, and Venezuelans, or the family reunification parole processes using Form I-134A, beneficiaries under the age of 18 must travel to the United States under the care and legal custody of their parent or legal guardian and provide documentation confirming the relationship.

If you are sponsoring a beneficiary seeking humanitarian parole, not applying under Uniting for Ukraine, the Processes for Cubans, Haitians, Nicaraguans, and Venezuelans, or the family reunification parole processes, you must submit a paper Form I-134, Declaration of Financial Support, along with Form I-131, Application for Travel Document, to the appropriate Lockbox location.
