Form I-800A Requirements: Complete Form I-800A, Supplement 1, Listing of Adult Member of the Household, for each adult residing in your household. Notably, this form is not required for yourself or your spouse (if married).
Consent to Disclosure: Submission of Form I-800A, Supplement 2, Consent to Disclose Information, is optional but allows us to share information about your case with your adoption service provider, home study preparer, or any designated individual or entity (excluding your attorney or accredited representative). Information generally cannot be disclosed to others without your explicit consent.
Request for Action: Form I-800A, Supplement 3, Request for Action on Approved Form I-800A, must be submitted to request an extension, change of country, or other actions post-approval of your Form I-800A and before filing Form I-800. E-Notification: Opt for E-Notification to receive confirmation via email or text message upon acceptance of your Form I-800A at a lockbox facility. Simply download and complete Form G-1145, E-Notification of Application/Petition Acceptance, and attach it to the first page of your application package.
